DECLARATION OF RESPONSIBILITY and WAIVER

1. Our participation in the “Hataw sa Tag Araw, Takbo para sa Pagbabago” at the Pacific Mall Lucena City, On June 18,2023, is made by our own free-will and no one forced us to participate.
2. I/My Son/daughter is already vaccinated and free from any symptoms of the covid 19 virus.
3. In any cases that arises in this competition regarding Covid 19, it will be my full responsibility for I declared that I/my son/daughter was free from any symptoms and vaccinated.
4. I will claim full responsibility in any untoward and unpleasant incidents pertaining to the physical and health safety of myself/son/daughter during this competition.
5. I am aware of all the protocols issued by the IATF regarding new normal, by always wearing a mask and cleaning hands with alcohol and sanitizer at all times to ensure the safety precaution of everyone.
6. I am fully aware that the organizer will not be responsible or in any way be liable for any death, disability, sickness, injury, loss of property or any other untoward incident during the course of competition.
7. It is my job to sustain the good behavior of my son/daughter during the event and I will claim fully responsible for the misconduct made by my son/daughter and not be dismayed if the organizer forced us to leave in the venue for the behavioral issues.
8. In any part of the meet that my son/daughter record a video or capture images just to make a bad publicity in social media, I will be in any ways take all the responsibility for this action and face all necessary trials by the organizers or the person involved.
